Individual Social Support Community Outreach Service AGED CARE
The Individual Social Support (ISS) service provides 1:1 support for those who are aged or prematurely aged, socially isolated, marginalised, and/or living in insecure housing. The service covers the Inner West council area.
The program offers home visiting, phone and web chat, and accompanied outings such as shopping, walks, and coffee. The focus of the service is to support individuals to remain independent and connected to their community, as well as identifying early when there is a need for a more intensive support through My Aged Care. Another key focus is on housing stability and, where required, enabling people to move to more secure and affordable housing.
The service is funded under the Commonwealth Home Support Program through the Department of Health until 2022.
Ideally, community members accessing the ISS service will be registered with My Aged Care.
