Individual Social Support (ISS) social connection for seniors AGED CARE
The Individual Social Support (ISS) service provides 1:1 support for those who over 65 (and over 55 for Aboriginal and Torres Strait Islander Peoples) living in the Inner West council area.
The program offers home visiting, phone and web chat, and accompanied outings such as shopping, walks, and coffee. The focus of the service is to support individuals to remain independent and connected to their community, as well as identifying early when there is a need for a more intensive support through My Aged Care.
The service is funded under the Commonwealth Home Support Program through the Department of Health and Ageing.
Eligibility
This service is for people aged 65 and over (50 years or older if you identify as an Aboriginal or Torres Strait Islander person), who live in the Inner West Sydney Local Government area
Must be referred for CHSP Group Social Support via My Aged Care
